KAYAK & STAND UP PADDLBOARD (SUP) RENTAL PROGRAM

Full Release of Liability & Financial Responsibility Form Full Release of Liability & Financial Responsibility Form Between the below named participant and the Three Belles, LLC and its staff members and event partners (their officers, officials, agents, employees, associates, volunteers, contracted individuals, event sponsors, advertisers, and owners and lessors of the premises and properties of Three Belles, LLC). The said staff members and event partners mentioned are heretofore referred to as the “Releasees.” NOTE: Read Carefully Before Signing In consideration for being permitted to participate in Three Belles, LLC’s Kayak and Stand-Up Paddle Rental Program, its related events and activities:

1) I have read all policies and have been informed of training that Three Belles, LLC provides for said rentals and programs, and understand that any and all risks of injury from the said activities involved in this program are significant and serious, including the potential for permanent paralysis and death, and while particular training, skills, preventative safety equipment, and personal discipline to utilizing such equipment and training/skills might reduce this risk from occurring, the said risks do exist and are a constant reality during participation.

2) I knowingly and freely assume all such risks described above in said participation, both known and unknown, even if arising from neglect of the Releasees or others, and assume full responsibility for my participation.

3) I willingly agree to comply with the stated and customary terms and conditions for said participation. If, however, I observe any significant hazard(s), whether natural or fabricated, I will voluntarily and immediately remove myself from said participation and immediately notify Three Belles, LLC staff members of discovered hazards.

5) I understand that I am responsible for returning all rental property in good working order and that I am financially responsible for any loss or damage cause to Three Belles, LLC’s equipment during my rental or events session. This includes, but not limited to any loss or damage to kayaks, SUP’s, paddles, pedal drive systems (for both kayaks and/or SUP’s), personal floatation devices (PFD’s), fishing equipment, and/or sailing equipment. I further acknowledge and understand that I am responsible for the costs incurred for any repairs and/or replacements for any lost or damaged equipment that occurs during the sessions of my rental or program participation, and that any financial deposits or holds will be acquired and utilized by Three Belles, LLC for financial compensation.

6) I understand that any and all personal belongings are my own responsibility and that Three Belles, LLC does not store and/or claim responsibility for any said belongings, including but not limited to cell phones, car keys, wallets, purses, personal floatation devices (PFD’s), paddles, or other outdoor gear provided by myself as the renting customer. Such items must be stored and/or maintained by my own person.

7) Any and all photos or marketing materials generated from my said participation are permitted for use in Three Belles, LLC social media and advertisements for the purposes of promoting Three Belles, LLC’s rentals, programs, and Outfitter’s merchandise.

I HAVE READ THIS RELASE OF LIABILITY DOCUMENT IN-FULL AND ACKNOWLEDGE AND FULLY ASSUME THE SAID RISKS AND LIABILITIES, AND THAT I AM 18 YEARS OF AGE OR OLDER UPON THE SIGNING THIS DOCUMENT. I FULLY UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND WITHOUT ANY INDUCEMENT OR EXTERNAL STIMULUS OR INCITEMENT.”

For parents and legal guardians of Participants that are legal minors under the age of 18-years (see below): This is to certify that I, as a parent/legal guardian with responsibility of the above-named participant, do grant full consent and agree to indemnify and hold harmless the Releasees from any and all liabilities incidental and a result of the minor’s participation and involvement in said rentals and programs as described above, even if arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law:
